What information do we collect?
If you read or download information, we automatically collect and store:
- the requested web page
- the domain from which you access the Internet (e.g. att.net)
- the date and time you access our Site or mobile application
- status of the request (e.g. success or failure)
- the type and version of your web browser (Firefox, Explorer, etc.)
- the operating system (Windows, Mac, Linux, IOS, Android, etc.)
The only way we obtain personal information through the Site is when you choose to provide that information to us, including through your access and use of Site (“Site Information”).
We do not respond to any questions concerning specific medical conditions. Notwithstanding the foregoing, if you chose to register or login as a user on a Site, you will be required to create or use a pre-existing user name and password. Any information made available through the secure Site is treated as confidential and certain protected health information made available through the secure Site is subject to the Health Design Plus Notice of Health Information Practices. HDP agrees to maintain any protected health information made available through the secure Site in compliance with its legal obligations pursuant to all applicable privacy laws, including, without limitation, the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), as amended from time to time, and its implementing regulations.
How do we use the information we collect?
Site Information we collect may be shared with Health Design Plus business partners and other organizations and individuals with which Health Design Plus has established a strategic alliance or other contractual relationship. The primary purpose behind the sharing of information is to better the services being provided to you by Health Design Plus and by the organizations and individuals for which Health Design Plus has established contractual relationships. Information will also be used to improve and carry out Health Design Plus sales, marketing, and business development efforts.
What about “cookies”?
“Cookies” are small pieces of information that are stored by your web browser on your computer’s hard drive. They permit HDP to recognize you and your access privileges on this Site, as well as to track Site usage. You can usually change your browser setting to prevent acceptance of cookies. Check with your Internet Service Provider for assistance in changing this setting. However, if you wish to access areas that are personalized for your use, cookies will have to be active.
